Project Name: Healthier West Wirral Lipid Optimisation Project
Project Summary:
The CWP is structured such that a multi-disciplinary health professional team will provide tailored care for patients, aligned with the Primary Care Network (PCN) Contract Direct Enhanced Service) DES and the Locally defined Cardiovascular plans.
The main objective of the CWP is to improve the quality of care for patients and support the early identification, review, and medical optimisation of patients with atherosclerotic cardiovascular disease (ASCVD)in response to the needs of the CW Partner.
Planned Milestones:
Collection of baseline data, in line with the above Project Outcome Measures & Data Collection table
Confirmation of clinical and operational pathway, policy and protocol creation, and readiness to begin the clinical activity
Collection & submission of 3, 6, 9 and 12 months clinical activity data.
Project Review meeting at 3, 6, 9 and 12 months to discuss project progress.
Development of business case and analysis of CWP data, submission of Final Project Report, Submission of Outcomes Summary

Anticipated benefits for patients:
- Improved access to lipid management care leading to optimal diagnosis and management of ASCVD treatments.
- Enhanced experience around ASCVD with ongoing management of the condition.
- Improved access to appropriate medication for suitable patients to preserve health and prevent long-term events
- Easier access to lipid management care closer to home in the Primary Care setting
- The additional capacity will provide additional time and support from PCN HCP with their lipid management, focusing on patients who may have previously not attended GP appointment or been lost to follow-up. Thus, leveling health inequalities within the PCN.

Outcome Summary:
Throughout the duration of the project, pharmacists have discussed lipid optimisation for all patients identified with ASCVD and with cholesterol levels which are not to target as per summary of national guidance for lipid management as per NICE National Guideline 238.
This project improved the quality of care, early identification, review and medical optimisation of patients with atherosclerotic cardiovascular disease (ASCVD).
Key Project Outcomes Data:
The collaborative working project led to lipid reviews of over 1500 patients, resulting in multiple, clinically effective interventions to manage lipid levels.
Outcomes:
820 patients have had a review with a Pharmacist and have had a follow up, review and lipid optimisation. This reduced the backlog of patients with ASCVD waiting to be seen by a pharmacist from 1427 to 607.
Patients with ASCVD and suboptimal lipids are identified and referred to the Pharmacist team by practice staff.
The project has been an effective vehicle for upskilling the clinical team as the projected deployed a multi-disciplinary approach to achieve the improved cardiovascular outcomes for the patient cohort.

Conclusion:
HWW practices will continue to provide lipid optimisation for secondary prevention in extended access time and integrated into Pharmacist clinic appointments.
HWW clinical pharmacists have gained the knowledge and skills through the Novartis project to optimise lipid management for secondary prevention patients in line with local and national guidance.
