Project Summary:
The main objective of the CWP is to improve the quality of care for patients and support the early identification, review, and medical optimisation of patients with atherosclerotic cardiovascular disease (ASCVD) who are at high risk of cardiovascular events and who are not achieving lipid levels as specified in national and/or local guidance, in response to the needs of the CW Partner. Ainsdale Medical Centre is the Lead Practice for the PCN. Novartis will contract with Ainsdale Medical Centre as they are the legal entity contracting on behalf of the PCN.
Planned Milestones:
- Collection of baseline data, in line with the above Project Outcome Measures & Data Collection table
- Confirmation of clinical and operational pathway, policy and protocol creation, and readiness to begin the clinical activity
- Clinical Audit and Clinical Audit Discussion with Member Practices
- Collection & submission of 3 months clinical activity data.
- Project Review meeting to discuss project progress.
- Collection & submission of 6 months clinical activity data.
- Project Review meeting to discuss project progress. Collection & submission of 9 months clinical activity data.
- Project Review meeting to discuss project progress.
- Collection & submission of 12 months clinical activity data.
- Project Review meeting to discuss project progress.
- Development of business case to continue the service offered by this project
- Analysis of CWP data, submission of Final Project Report, Submission of Outcomes Summary

Anticipated benefits for patients:
- Improved access to lipid management care leading to optimal diagnosis and management of ASCVD treatments.
- Enhanced experience around ASCVD with ongoing management of the condition.
- Improved access to appropriate medication for suitable patients to preserve health and prevent long-term events Easier access to lipid management care closer to home in the Primary Care setting
- The additional capacity will provide additional time and support from PCN HCP with their lipid management, focusing on patients who may have previously not attended GP appointment or been lost to follow-up. Thus, leveling health inequalities within the PCN.

- Increased proportion of ASCVD patients reviewed by primary care
- Increased proportion of ASCVD patients receiving expert and timely review closer to home
- Reduction in ASCVD referral rates to secondary care

Outcome Summary:
This collaborative project set out to improve the quality of care for patients and support the early identification, review, and medical optimization of patients with atherosclerotic cardiovascular disease (ASCVD) who are at high risk of cardiovascular events and who are not achieving lipid levels as specified in national and/or local guidance, in response to the needs of the CW Partner. The key project data is set out below, however this project terminated early due to capacity challenges which prevented the full collection of data as set out in the executive summary, these challenges also prevented the development of business case to continue the service offered by this project.
Key Project Outcomes Data:
Number of patients not on a statin therapy: Baseline – 46, at the end of the project – 41.
Number of patients on a suboptimal statin therapy: Baseline – 27, at the end of the project – 18
Number of patients on sub-optimal statin LDL-C levels despite maximum statin: Baseline – 99, at the end of the project – 36
Number of patients who have attended a review appointment as part of this project: Baseline – 0, at the end of the project – 93

“From my experience of completing the project it was a great opportunity to have a dedicated clinic with patients purely to discuss their lipid management. In particular patients are often just given medication after an event with little explanation as to why and what they are taking - this project allowed a good discussion with patients on why they were selected ( ie; high ASCVD risk) what the rationale for lipid management was - review of evidence and latest guidelines on treatment goals and then discuss the implementation and rational for treatment adjustment to hit the target - which the majority had never understood before and so had the opportunity to be much more directly involved in ownership of their health and treatment objectives - I think this aspect of the project was as valuable as medication adjustment and ultimately hitting the target goals.”

Conclusion:
This ASCVD Lipid optimisation project achieved the main objective of early identification, review, and medical optimisation of patients with ASCVD who are at high risk of cardiovascular events and who are not achieving lipid levels as specified in national and/or local guidance for those that that were able to be reviewed prior to the early termination of this project. Unfortunately, the capacity challenges experienced by the Collaborative Partner prevented the development of business case to continue the service offered by this project.
